Academic Warning & petitions

Forms for if you’re on academic warning or need an exception to academic policy.

• Academic Warning Worksheet (Second Warning)

  Complete the Second Academic Warning Worksheet if you have been placed on Second Academic Warning. You will be required to meet with an academic advisor to complete this worksheet. Submit the worksheet to Admissions & Records by 5 p.m. on Monday of Week 2 of the following term.

• Academic Reinstatement Petition (Third Warning)

  Complete the Third Academic Warning Petition if you have been placed on Third Academic Warning. You will be required to meet with an academic advisor to complete this Academic Reinstatement Petition. Submit the worksheet to Admissions & Records by 5 p.m. on Monday of Week 2 of the following term.

• Student Petition Forms

  In cases of an exceptional circumstance, you can request an exception to a published academic policy by submitting the petition form below. 

  • • Current Student Petition Form (active student account)
    • Non-Current Student Petition Form (inactive student account)

Enrollment Status

Forms related to completing or changing your academic path.

• Special Admissions - Under 15 Approval Requirements + Form

  If you are under 15 years old and interested in attending COCC, complete the Special Admissions - Under 15 Approval Requirements + Form.

• Term Change Form

  • To access this form please login to your application
  • Select your old application (must be labeled as submitted)
  • Select “Need to Change your Term?”, complete the questions, then hit submit
  • Please note that term changes take 1-2 business day to process

• Veteran Status Form

  If you are a current or former member of the military, complete the Veterans Status Request Form.

Grades & Degrees

Forms related to grades, courses, or completion of a degree/certificate.

• Application for Certificate or Degree

  To apply for any degree or certificate that is not your declared program of study, or if you have enrolled in the final course(s) for your degree or certificate, please complete the Application for Certificate or Degree Form. 

• Challenge Course

  You may be able to earn credit for what you already know through a Course Challenge.  If you are interested in challenging a course you must receive permission from the instructor and department chair, and complete the Petition to Challenge a Course form.

• Degree Status Change Form

  If you would like to change your degree seeking status, please complete the Degree Status Change Form.

• Incomplete Grade Contract
  If you are requesting an Incomplete Grade due to extenuating circumstances that would prevent you from completing your class, please complete the Incomplete Grade Contract.

Student Information & Privacy

Forms to manage how your information is shared or listed on your account. 

• Legal Name Change Form
  To change your legal name and have your COCC student records updated, please complete the Legal Name Change Form.
• Non-Disclosure of Student Information Request Form

  If you do not want COCC to release directory information to anyone, please complete the Non-Disclosure of Student Information Request Form. This form prohibits COCC faculty and staff from discussing any student business over the phone and all business will be conducted in person only.

• Non-Disclosure of Student Information Request Removal Form
  If you would like to remove your request for Non-Disclosure and allow directory information to be released, please complete the Non-Disclosure of Student Information Request Removal Form. 
• Release of Information Request Form
  To grant COCC permission to release your information to designated individuals, please complete the Release of Information Request Form. 
• W-9S
  To provide your tax identification number, please complete the W-9S Form.
